Requirements: Four (4) copies of the Certificate of Live Birth duly accomplished and signed by the proper parties; Affidavit for delayed registration by the father, mother, guardian, declaring therein, among other thins, the ff: name of child date and place of birth name of the father if the child is illegitimate and has been acknowledged by him if legitimate, the date and place of marriage of parents; and reason for not registering the birth within thirty (30) days after the date of birth

Registration of Live Birth |

The birth of a child shall be registered within (30) days from the time of birth in the office of the Municipal Civil Registrar where the birth occurred, except in the following cases: if the parents of the child are both foreigners but residents of the Philippines, the birth shall be recorded in the municipality where the mother habitually resides in the Philippines Description – this pertain to an Act No. 3750, otherwise known as the Civil Regitry Law which took effect on February 27, 1931, wherin acts, events, status of persons shall be recorded. The Civil Registry embraces all acts of civil life affecting the status of persons and is applicable to all persons residing in the Philippines. - BIRTH CERTIFICATE/LIVE BIRTH This is the complete expulsion or extraction of a product of conception from it mother irrespective of the duration of pregnancy, and shows any other evidence of life Procedures: present the marriage contract of parents fill-up the information sheet form proceed to Mun. Treasurer's Office for payments Requirements Marriage Contract of Parent Residence Certificate of Late Registration Official Receipt Fees: P30.00 for the newly born P5.00 per year + P40.00 minimum surcharge for late registration
